An investigation has been launched at the Sebastian County Detention Center after authorities say an inmate was released by mistake.

The inmate, Casey Lambert, mistakenly released from the Sebastian County Detention Center before his Arkansas Department of Corrections hold was satisfied, according to Phillip Pevehouse with the Sebastian County Sheriff’s Office.

Lambert was at District Court in Fort Smith Thursday, according to Pevehouse.

"When he came back he was released from our detention facility," said Pevehouse. "However, there was still a pending Department of Corrections hold in place. The error was discovered and immediately our office began to work on getting him back."

Lambert was arrested and taken back into custody later Thursday afternoon along the 2100 block of Grand Avenue, according to Pevehouse.

Now, deputies are working to figure out what happened.

"Right now it's under an internal review in which we are trying to figure out exactly what occurred," Pevehouse said. "Some of the things we've looked at so far is that we know that there was at least one error that was made when he should've been reclassified by our current filing system when he became an ADC hold."

The investigation is ongoing.

"Our goal is to be a hundred percent." Pevehouse said. "We want to be getting it right every day and we're going to keep working toward that goal. The more technology we have in our favor certainly is going to reduce the level of human error."

Lambert is being held at the Sebastian County Detention Center and will soon be transferred to the Arkansas Department of Correction.
