FORT SMITH, Ark. — Mercy just opened its first 65 Prime+ clinic in Fort Smith that will focus on elderly patients.

The new 4,571-square-foot clinic opened on Oct. 1 and had its ribbon-cutting ceremony on Oct. 4.

65 Prime+ is at 6801 Rogers Ave. suite 107, which is the former Convenient Care at Mercy Tower West. Mercy said this clinic is designed for patients 65 years or older and will have "an expanded primary care experience with longer and more frequent appointment times." This allows patients can have plenty of time to discuss their health.

Providers for 65 Prime+ "are supported by a registered nurse and care navigator," the clinic said. The clinic will "provide clinical support to patients during visits and care between visits, while the care navigator helps coordinate all other aspects of the patient’s care," according to Mercy.

Those services may include:

  • Referrals to specialists, therapists or other providers
  • Setting up tests or procedures
  • Obtaining prescriptions and medical equipment
  • Ensuring all preventive care such as immunizations, lab tests and cancer screening are obtained.

Other services:

  • flu shots or other immunizations
  • medicare annual wellness visits
  • general medical evaluations
  • wellness exams
  • minor illnesses and injuries (strains, sprains or fractures)
  • monitoring chronic conditions (diabetes, hypertension, incontinence, osteoporosis, high cholesterol)
  • care for Alzheimer's disease, dementia or early memory loss
  • Parkinson's disease and other movement orders.

Additionally, the clinic will have reclining chairs, reserved parking and a covered drop-off area.

To make an appointment at 65 Prime +, patients can call them at 479-274-3350.
